Some states have a licensure expiration date based on date of birth or other criteria. My nursing license (not MD) which is also supposed to be good for 2 years, expired slightly more than a year after my initial licensure date because of that. I'm sure it's the same scenario for you. In fact, this is what the MD BON states:
Renewal of CNA CertificationCNA certification is generally for a 2-year period following the date of the first renewal. The CNA certificate expires on the 28th day of the individual's birth month in an odd or even year. If the individual is born in an odd year the certificate will expire in an odd year. If the individual is born in an even year the individual will expire in an even year. Costs for renewal is $40 for the 2-year period.

They also state that working for 16 hours in the 2 year period before renewal is also a must:
Active Practice RequirementAll nursing assistants must have actively practiced as a nursing assistant for 16 hours in the two years immediately preceding the renewal.

Whether you have to retake the course is a question best answered by the BON, but from what I've read it appears you are not eligible for renewal unless you work 16 hours between now and the date of expiration.
