Angiotensin 2 blockers and hypotension


Has anyone besides me noticed an increased incidence of severe hypotension resistant to most pressors after induction with pts who took their arb's the day of surgery. I had to cx a case yesterday because of this.

Here's a link which (under the section about ARB's) cites a couple of studies that showed just what you're referring to.

I have recently done several pubmed searches regarding this and it seems the research shows vasopressin is the treatment of choice. There are several articles from 2005 and 2006 comparing Epi, norepi, Vaso, ephedrine, Neo and Methylene Blue. Vaso was superior in all studies.
