I understand the midwifery model of care when it comes to pregnancy and labor, but I am curious about how gynecological care is addressed? Are there any midwives that can share information on well woman care and family planning?

It doesn't really differ from what a gyn would offer. Family planning options including pills, patch, ring, diaphragm fittings, IUD insertions, etc. The surgical options (Essure, tubal ligation) would require referral to a surgeon. Pap smears, STI screening, breast exams, etc are all performed by midwives. Well woman exams are a great opportunity for teaching and patient advocacy and are midwives are ideally suited for it with a focus on wellness and patient education.
