Just copy and paste this 2 more times on your word document so you will have 3 columns; one for each patient. I used the table format that adjusts the cells as you type for each section. Hope this helps,

Here is the one I use, I do not know what kind of unit you are on, you probably posted it but I did not read, If you like it just cut and paste it to your word and you can revise the areas on it that you want. Actually it is cut and pasting a little funny, email me and I will send it to you

I work SICU so my patient's are slightly different from the avg CCU patient, but we do get post op CABGs.

Anways here is one I got and altered to fit my needs...
