Code Blue Flowsheets

I am looking for examples of flowsheets for recording code blues. We are very good at recording the events, but have not yet found a concise mechanism for recording the event/action taken/rationale/result. Since we will most likely be revising our current flowsheet entirely, I would welcome any and all examples.
