IABP removal

  1. Has anyone out there removed an IABP or seen another nurse do it. A RNFA or APN. Appreciate any help. I am a CRNFA(certified registered nurse first assistant) in cardiac surgery. The surgeon I work with wants me to remove them when the patient no longer needs it.

In my facility (in PA) only Physicians are allowed to pull IABP's. Check your Nurse Practice Act to see if this is within your scope of Practice.

Are you referring to balloons or arterial sheaths? As an RN in Pennsylvania, I am permitted to pull arterial sheaths but NOT balloon pumps.

In NC, the MD pulls, and the trained RN holds pressure, evaluates the site and dresses it per policy. Now I know of many states that have IABP removal check offs as part of catatory 2's per nursing board of licensure. requires a trained RN with 3 check off's to do this.

In many states the nurse tech holds pressure after didactic training as well as three check offs per year

Where I work only the physician removes a balloon. Femorial arterial sheaths us RNs remove.

    To clarify:

RN's pull and hold arterial sheaths. Cath lab techs (who are radiology techs w/add'l training) pull and hold arterial sheaths and IABP balloons. MD's can do it all.

I am a cath lab RN, and we pull all sheaths except the balloons. Only the MDs can pull balloons with us then holding pressure.

I pulled a baloon the other day, but the lady had just died..............***Our MDs always pull baloons on live patients. They also pull swans if the patient has a PPM or TPM. Also some MDS pull swans on a patient with a LBB, just in case they put them into CHB and give them a RBB...........

I did a removal in the 1980's before my BON decided RN's were not to do this. Our MD's or their SA/PA's do it & we hold pressure & all the other stuff involved. I am just glad for one LESS thing to do, we do so much else!!

In my facility the MD pulls the BP, but RN pulls pretty much anything else including sheaths, swan, cordis, central line, whatever.
