From the rehab floor to case management...

I graduated in January with my lvn, Ive been working at a skilled nursing facility. I just had a baby 4 weeks ago, and am making a career change to case management. The hours are better for me with the new baby, but I would welcome any advice on the job. It seemed like a good job to have while I'm trying to get my RN as well, any thoughts on cm? Also its strictly by phone, which will take some getting used to on my part since I'm used to in your face patient interaction....kinda nervous!
