HELP! Discharge Planners/Case Managers


I am a RN consultant to the medical device industry, & I am hoping to gain a better understanding of a few home care issues. I am conducting a telephone survey of discharge planners/case managers. I am interested in learning more about how supplies get paid for when the patient is discharged from the hospital. I am looking for experienced discharge planners/case managers who are well versed in insurance issues, both Medicare and commercial insurance payers.
