Emergency Response Team/Case Managers

I am compiling a note book for my leadership class. Are there any case managers out there that can help me? For my leadership project I am putting together a resource book that would benefit a case manager on a emergency response team. I can not go too deep as I am doing all 50 states. I am looking for state agencies but my question is: is there something special that I need to have in it? I was thinking of having phone numbers and web sites of the organizations that help people in time of need.

Any ideas that would benefit a case manager?

Thanks
Susan
