DM to CM


I was curious if anyone has ever changed from Disease Management to acute care Case Management? I am doubting I will get a reply but thought I would post my questions anyway.

I have done the reverse, gone from case management to disease management. In the acute care setting, I would think they would be quite similar as you are managing compliance,treatment, discharge planning and such. My $0.02.

IT really depends on the environment that you are doing case manaement in. I work in the Insurance environment and went from ICM(Acute CM to DCM) found DCM to be much more educationally focused but encompassing the Acute CM aspect as well. Once handling the acute situation(managing d/c needs setting up hh and then once back home the real education from the disease managemet perspective kicks in to try to prevent a reoccurrence to the acute. DCM has been most rewarding.
