DCP/Choice Consent Form


Hospital Case Managers: Do you utilize a consent form for your pts to sign when confirming their choice for ie, home health agency, SNF, etc? Is this form an integral part of the chart? If yes, then what does your form include? We are looking at making changes to our existing one.

We have the standard lawyer initiated stuff on the front, you know that you are being offered a choice . at the top of the form, it lists DME?Home Health. We then asked them to put on the back the lists of all dme and hh in area, so patient can look, pick their choice, and its signed and witnessed. evidence of pt getting their choice right there on back of form. hope this helps

We have a patient choice form for our Medicare patients only. If the Medicare patient does not have an agency we offer them a choice, and if they can't make a choice, we ask if we can arrange it through the hospital's home health agency.
