Our CM department is looking into different CM modules and how other facilities are managing their programs. Currently, we have CM's and SW's. The case mgrs handle discharge planning for the pt.s, assign appropriate cases to the SW's..ex: cases needing hospice, SNF transfers that need to have PASSR's or ULTC100's done. The case managers are also reviewing for Utilization and checking for appropriate status orders, calling in insurance reviews. In other words wearing a multitude of hats.
We would like to have anyone's input on how their modules work? Do you utilize the triad system, having specific RN's who are in charge of Utilization Review and RN's who work closely with the SW's with discharge planning? Or, do your CM's handle everything from UR to DP? If you have a different model I would most certainly like your input?
We also have a Case Manager in the ED full time (me) during the days and are trying to find one for the pm shift. Our ED cm model is a whole different story, we where so many different hats.
