Avoidable days/Concurrent Documentation


I am a RN working in a hospital CM department. The CM in the department do UR and discharge planning. I have 2 questions.

The first is how do you manage avoidable days that are attributed to a physician? Do you require the physician or medical adviser to review these prior to attributing these to a physician?

We are starting to have the CM on the units do concurrent docuemention in order to "catch" the documention by physician on the medicare never events. Is anyone else doing this in addition to UM and dc planning? Or are you using a documentation specialist for this function?
