by jenn1970 Dec 10, '12I just accepted a hospital based case manager position in a large teaching hospital. I will primarily be responsible for D/C planning. I am a diploma grad with 20 years bedside experience. Any advice for this rookie?? Thanks!

Also, if possible make sure your job description reads more like a case manager than just a basic discharge planner. Case Management is more than simply coordinating discharges to home or to a nursing home.
If your hospital has access to them McMillan has a good set of tools for determing case management outcomes too.
