    I just accepted a hospital based case manager position in a large teaching hospital. I will primarily be responsible for D/C planning. I am a diploma grad with 20 years bedside experience. Any advice for this rookie?? Thanks!

  3. Study up on Case Management. A good resource is the Case Management Society of America Standards of Practice. It's a free PDF download.

    Also, if possible make sure your job description reads more like a case manager than just a basic discharge planner. Case Management is more than simply coordinating discharges to home or to a nursing home.

    If your hospital has access to them McMillan has a good set of tools for determing case management outcomes too.
