    Can someone help me with this...I am licensed in Missouri and I have accepted a summer camp nurse position in Pennsylvania. I've downloaded the "application for licensure by endorsement/temporary practice permit" form from PA Board of Nursing. It has two choices..1) License or 2) License and Temporary Practice Permit.
    Since the position is only temporary I assume I want the 2nd option.
    Do I have the right form?

    You want the first option. The temporary practice permit is for recently graduated GN's that have not taken the NCLEX yet.

    Keep your receipt. Your camp should pay any fees.
