Below is a copy of the checklist I made for the application process.

Maricopa County College District Nursing Program Application Checklist:

  • Call Arizona Department of Public Safety, 602-223-2279, to request a Fingerprint Clearance Card Packet, get fingerprinted, complete form from packet, and mail form with fee to DPS. They will mail you your fingerprint card (a plastic card the size of a credit card or drivers license). Copy the front and back of card to submit with nursing school application. This process takes weeks, so do it as early as you can.

  • Get a TB skin test done (PPD), and make a copy of proof of test to submit with nursing school application. Don't forget, you will have to return in a couple days for the test to be read and signed off.

  • Complete the Health & Safety Documentation Checklist and make copies of all your documentation to submit with your application.

  • Take the NET test, and get a printed copy to submit with nursing school application. You can get the study guide, "The Net, The Hobet, The Help Entrance Tests" by Frost & Jarvis, at Amazon.com for about $25. This is about half what it costs at the MCCD school bookstores.

  • If you don't already have it, get CPR/BLS certified and make a copy of the front and back of your card to submit with nursing school application.

  • Complete the MCCD Nursing Program Application

  • Make sure you have a completed Student Information Form on file at your college of first choice (the place you will turn in your nursing application).

  • Have Official Transcripts form ALL colleges/universities attended sent to your college of first choice.

  • Get copies of all your unofficial transcripts to take to your school's advisor for evaluation. Most schools will allow you to sign into their website and download these for free.

  • BE SURE YOU MAKE A COMPLETE COPY OF YOUR ENTIRE APPLICATION PACKET BEFORE YOU SUBMIT IT.

  • See an advisor at your school of first choice to check for completeness of your nursing program application. You should be able to submit the application at this time. Take ALL your stuff to this meeting.

Another good idea is to get up to date on your injections and vaccinations, otherwise you'll be struggling to get all of them done in a short time. Here are the ones you will most likely need. All healthcare providers and workers need these.

-Td or Tdap if it has been 10 years since your last

-Hepatitis B series

-Positive IgG (Varicella) titer if you have had chicken pox while younger and never received vaccinations. Otherwise, provide proof of two Varicella vaccinations.

-Another MMR if you only have had one to make a total of two. You can also get a titer of MMR instead.

-a two step TB skin test (within the last six months at placement time)

I got these from a checksheet after I was accepted into the program. They are only giving a month. It's a lot to get if you are not up to date. Fortunately, I was, so no biggie for me, but I know others will have to scramble.
