Peds office having issues

I am an RN in a busy pediatric office. I was hired on to do telephone triage, but i do alot more now. We have alot of problems with flow and getting documentation complete and accurate. Also, the vaccine reconciliation is always off. The nurses count the vaccines 3 times a week and seem to always count wrong. Anyone have any ideas, how to you make an office flow well. Our back office is composed of LPNs and MAs, does that make a difference?
