PRC form and requirements for CES Request for Validation of License

Hi , dear fellow nurses in the philippines,

I will be so glad if someone could share a copy of the form to fill in the prc when you process your CES Request for license Validation for CGFNS . Or is there any to fill in? Are there requirements to present to PRC when you submit the CES Request For Validation of License ? Your inputs will be greatly appreciated.

You use the form that is included with the application for the CES, no other form is accepted by CGFNS for this.

But you also will need to include the fee that they charge for completing the paperwork, they never do anything for free.

Hi Suzanne, Thanks for the reply. I have that form from the CES. I am just anxious whether there are copies of transcripts,diploma or license needed when I have the CES form brought to PRC, as I will have my sister do it in my behalf.

By the way, thanks so much for your help passing the nclex rn!

Those items should have already been submitted for the CES, the only thing that the PRC needs to do is validate that one form.

PRC only validates your license using the form from the CES application form.

The transcripts are to be sent by your nursing school, in w/c another CES form specific to it is needed to be given to the School's registrant together with a certified true copy of the transcripts. You pay extra for the certified true copy per your school's fee schedule and you pay on the manner of how the school send those documents.

The nursing diploma has not been required anymore when there is already a transcript and a certified license coming from the school and PRC. And if you already have previously submitted your transcript to CGFNS before, you don't have to submit it again.

Just curious... why you did not do this yourself before going to the US?? And what State BON are you applying for licensure?

i have applied for a reissuance of my ces report for the state of pa. i am currently registered for the sc state board of nursing and pa requires ces for license endorsement thus i am into ces processing at this time. and, thanks for the congratz!

congrats again. all you need to do now then is the local license validation from prc then. it is relatively easy. you need to give some sort of letter of authorization to your sister though and try to send it via dhl to be sure. prc already allows this and actually have permitted dhl to have a station just in front of them. you need only to pay extra for it.

Hi guys!

I have a question about the request for validation of registration/ license for the CES.

If I dont have a local license (havent taken it and no plans in taking it), what could I do regarding this requirement then?

But be aware that many states in the US are now requiring the local license before they will permit one to sit for the NCLEX exam and noticed that you are in CA, and be aware that CA has now gotten very strict on this ruling.

If you have a green card, you are still a citizen of the Philippines, and are not exempt from this requirement per the BRN.

And if you are in CA, why are you getting the CES done as they do not require it?

Yes I am aware. I am one of those unfortunate ones who havent been granted the eligibility because of the new requirement implemented (the local license)in CA. Regarding that I'm feeling down:down: because I've read some of the users here have been granted their eligibility even if their application date was on December 2007(same as mine) and I have not been eligible. It is just so unfair .:crying2:(sorry for venting it out)

Anyways,I'm moving on so Im applying in Nevada. :sniff:(sigh start from scratch)
