- Sep 13, '08 by thinkerbellinkRN, can you please PM me your e-mail add, i have some enquiry too. thanks!
- Sep 14, '08 by kit25Quote from linkRNHi! You might have misunderstood me. They've received my application last Aug. 15, 2008 and I was able to receive the acknowledgement receipt last Aug. 27, 2008. With your question on transcript, yes, the notary public notarized it at the back of the document.@ kit25
I have a question about some attachments in the application: Did the notary public or school registrar certify your TOR to be true copy of the original? Plus notarized it at the back?
PM me your e-mail so we can discuss this thoroughly. I have not undergone the part A yet. I am also a June 2006 passer and explained this to the Council. Well, they mailed me a letter of acknowledgement for explaining the circumstances with the June 2006 licensure examination. I have also notarized my VisaScreen, CGFNS Cert. and US Board Cert and license to practice and sent them all along with the application packet. Guess what? I was chosen for a criminal background audit and have to send them a notarized copy of my NBI clearance. My name was sent to the QLD police Service but reassured that this will not delay the processing. Its fine with me. I don't have skeletons to hide. That's why I laid all the cards. - Sep 15, '08 by punjabiQuote from kit25to kit25:Hi! You might have misunderstood me. is it okay for the contact# and address to be handwritten on the docs or should it be stamped just like the name of the notary officer and date?
did u submit it thru the usual postal mail or courier? courier arent accepting it if you're to submit it to qnc POBOX address... correct? in the applix form, there's an adress: Level 14 charlotte st. ... not so sure if u got across with it.. but isn't considered as a physical or alternate address?
is it okay to send it to that address thru courier so that it wld,ve been faster?:stone - Sep 15, '08 by kit25Quote from ianoid182Hi. My lawyer notarized it the way they are notarizing most Philippine documents (telephone numbers and addresses are not included). I have personally inquired this with an agent from ANSM and said that it would be fine if you don't include the address and telephone numbers. Most of their nurse applicants were accepted. Howver, I suggest that you still input the adddress and telephone numbers as requested. As of now, I don't have any notifications coming from them that I need to supply these information. I would be glad to present them my original documents also for verification purposes.to kit25:

I sent the application via registered mail. It took about 15 days. If you have enough, you can use the postal service's EMS. That's roughly 600 pesos for Australia and it will take about a week to arrive in the PO box address. I strongly, recommend that you use the PO box address, because this is the address for correspondents. I think the Charlotte address is used when you have to visit their office.
I will update the forum about the whereabouts of my application. Please let me know if you have questions.javRN likes this. - Sep 16, '08 by punjabiQuote from kit25thanks for the info...really appreciate it....Hi. so, it's okay that address and phone# are handwritten? - Sep 17, '08 by nursemaviI applied for registration with NMBWA. When I had my docs certified by a notary public, I just wrote the telephone number of the lawyer since his stamp does not indicate his tel. #. My docs were received and evaluated already. I already got a decision from NMBWA which means handwritten telephone number is acceptable to them.
Good luck and God bless.javRN likes this. -
