I recently applied for my DEA license as a nurse practitioner and just wanted to know what to do next regarding schedule II medications. I know I need to fax a copy of my license and a copy of prescriptive authority to the state agency (in detroit). Is there any form that I can use online? I think I basically need my supervising physician's signature but the detroit office doesn't answer my phone calls and I don't know if there's any form that my supervisor can sign. Or should I make my own letter saying my supervisor delegates prescriptive authority?
What have you or other NPs done in Michigan? My classmates' companies have done for them and my company's starting to grow so I just don't know what exactly I need to do.
